• Arrange for services

• Follow up with you to see that services were obtained

Our Staff Will Also:

• Assess needs and resources and evaluate medical concerns, daily activities and caregiver support

• Assist in developing a plan to help Seniors continue to live at home

• Continue contact with family/caregivers to ensure needs are met

• Provide emotional support to the client and caregiver
